Family Services

Working closely with individuals, their family and Support Coordination, The Family Services Department of SBR, proudly provides In-Home and Community, Companion, Community Participation, and Respite Support to over 300 individuals and their families throughout the city of Philadelphia and its surrounding counties.

In-Home and Community Supports

This is a direct service provided in both home and community settings to assist individuals in acquiring, maintaining and improving self-help, domestic, socialization and adaptive skills necessary to reside in the home and community-based settings.

Services consist of support in the general areas of communications, self-care, fine and gross motor skills, mobility, socialization, relationship development, personal adjustment and the use of community resources.

Community Participation Support (CPS)

The service of Community Participation Supports is intended to facilitate long-term, sustainable relationships, activities, and engagement with others in the community. This service is provided on a 1:1 basis in the greater community.

Companion Services

Companion services are provided to individuals living in private residences for the limited purpose of providing supervision and the necessary care and minimal assistance that is focused solely on the health and safety of the adult individual.

Respite Services

Respite services are direct services that are provided on a short-term basis due to the absence or need of relief of the usual caretaker.

The Family Services Team at SBR is dedicated to providing exceptional in-home and community-based support to the 250+ individuals currently receiving services.  With an emphasis on individual choice, the Family Services Team advocates for and encourages all who receive services to live a meaningful, everyday life.

The various services provided by SBR are waiver funded through the PA Department of Human Services, in accordance with Philadelphia Intellectual disAbility Services (IDS).  In order to receive services through SBR, a referral must be made by a Supports Coordination entity.  For more information on how to select SBR as a support provider, please contact us 215-333-2280.
