Information accessed and its purpose
HCSIS Plan Summary helps authorized staff read selected information from an HCSIS Individual Support Plan already open in their browser.
When activated by the user, the extension reads relevant page content, including identifying information, medical assistance identifiers, dates of birth, addresses, diagnoses, plan dates, service information, related outcomes, and coordinator contact information. It uses this information to display a readable summary, provide links to source locations, and support user-requested printing.
The extension does not provide additional access to HCSIS or bypass its access controls.
Local processing and storage
Extracted information is processed locally in the browser and held temporarily in memory and displayed views. The extension does not maintain a database, record history, or persistent cache of plan information.
The only settings saved by the extension are the selected service-field identifiers and the preference controlling service-detail reference links. These settings are stored locally and contain no plan values or personal information.
Sharing and external actions
The extension does not send extracted plan information to SBR servers, analytics services, advertising services, or AI services. It contains no telemetry or analytics and does not log extracted information.
Printing is initiated by the user. Chrome may pass the summary to print preview, a printer, or a saved PDF. These outputs are outside the extension’s control and must be handled according to SBR’s information-security and records policies.
Clicking a coordinator email link passes the coordinator’s email address to the configured email application. The extension does not add a consumer name, subject, message body, or other plan information, and does not send an email automatically.
Retention and user control
The extension clears its displayed summary when the source context changes or the extension page closes. It does not retain plan information as persistent application data.
Saved display preferences remain locally until changed or the extension is uninstalled. Uninstalling the extension does not remove the original HCSIS record, printed documents, saved PDFs, or information retained by other applications.
Limited use
Information accessed by the extension is used solely to provide its stated summary, source-navigation, printing, and coordinator-email features. It is not sold or used for advertising, profiling, credit decisions, or unrelated purposes.
The extension’s use of user data adheres to the Chrome Web Store User Data Policy, including its Limited Use requirements.
