Home and Community Habilitation

The goal is to support individuals with developmental disabilities by encouraging their independence and social development across all settings within the community. The services under Home and Community Habilitation are typically inspired by the individual, the family, and the related needs as determined by the collaboration of the individual, the family and the team. SBR staff, are available to assist with maintaining hygiene, household maintenance, shopping, budgeting, medication management, time structuring, assisting in the pursuit of a healthy lifestyle (food choices and exercise) and in establishing social relationships.

Additionally, staff offer support to individuals by connecting them to natural supports in their community.
